The Idea
Most medical records are built around the institution that holds them. Patient X Project flips that model. Every record starts with the patient — or their legal guardian — and it stays theirs: to edit, share, or delete, at any time.
PX combines two kinds of information into a single chronology. Foundational details are entered once and updated as needed — profile, current medications, hospitalisation history, immunisation record, reproductive history. Recurring check-ins are completed as often as the patient likes, building a timeline of how they're actually doing, not just a snapshot from the last appointment.
The result is a health record that reflects a life, not just a file.
Who's in your circle
PX is built around relationships that the patient chooses and initiates. Nobody is added to a record without the patient's say.
The record owner. Initiates every relationship, holds full edit and submission rights, and can add, remove, or change anyone else involved. If there's no guardian, the patient can delete the record and close the account.
Holds the same rights as the patient for legal or care reasons. Can be assigned by the patient, or can set up the record on the patient's behalf and assign guardian status to themselves.
The healthcare provider — recommended for every patient. Can view, chart, download and print assigned records, and leave notes that trigger an immediate notification to patient and guardian.
The traditional emergency contact. In the event of death, where no guardian is assigned, the next of kin becomes the record's owner — ensuring continuity of care and information.
Designed to be understood
A picture says a thousand words — and when you're unwell, that matters more, not less. PX dashboards are built to be clear for everyone, including people managing advanced illness, older adults, and anyone for whom fine print isn't easy to navigate.
Each check-in captures a simple health snapshot across four areas, scored from 1 (lowest) to 10 (highest) on an intuitive colour scale. As more check-ins are completed, PX charts the scores over time, with hover-to-explore history so the bigger picture is always visible.

Trust, first
Before any medical information is collected, PX asks for informed consent — in plain language, not buried fine print. The patient remains the owner of their data at every step: retrieve it, add to it, or delete it at any time, and choose exactly who else can see it.
Where information contributes to broader reports or analysis, it is always de-identified first. Data is hosted on third-party infrastructure compliant with medical data handling standards such as HIPAA.
